Jack A. Polk, age 80, passed away Thursday, June 4, 2015, in North College Hill, Ohio. He was born November 7, 1934 in Cincinnati, Ohio. Visitation is from 11:00 am until 12:00 pm, on Friday, June 12, 2015, at the St. Mark AME Zion Church, 9208 Daly Road, Cincinnati (45231). The funeral service will immediately follow the visitation beginning at 12 noon. Interment is in Crown Hill Memorial Gardens at Colerain Township, Ohio. You may express online condolences at www.HerbWalker.com. Walker Funeral Home of Cincinnati is serving the Polk family.
