Donna Maria Hazel-Smith, age 59, passed away on Friday, December 21, 2018, in Cincinnati, Ohio.  She was born October 6, 1959, in Chicago, Illinois.

Visitation is from 9:00 am until 11:00 am, on  Saturday, December 29, 2018, at the Walker- Walnut Hills Chapel, 1025 E. McMillan Street , Cincinnati  (45206). The funeral service will immediately follow the visitation beginning at 11:00 am.

Interment will follow at Spring Grove Cemetery, 4521 Spring Grove Avenue, Cincinnati, Ohio 45232.
